Antibodies against citrulined vimentin


Anti-citrullinating anti-Saq / anti-MCV antibodies are representative of a wider family of anti-citrullinated protein / peptide autoantibodies (ACPAs). Cytrulinated vimentin is one of the eliminated (citrullinated) autoantigens of the synovial membrane, considered in the consideration of the pathogenesis of rheumatoid arthritis (RA). According to some authors, autoantibodies against citrulline-positive VMS are a promising serological markers for RA, although they have not been found in routine diagnostics. The first and so far the only Polish study on the utility of anti-Sa autoantibodies in the diagnosis of RA did not show any diagnostic benefit from the anti-Sa anti-Sa quantitative evaluation in addition to routine anti-CCP autoantibodies. The diagnostic sensitivity of anti-Sa in the diagnosis of RA was 36.6%, and the diagnostic specificity was 96.9%. Anti-Sa and anti-CCP co-occurrence was reported in 36.6% of RA patients and no isolated case of anti-Sa was demonstrated. According to the cited work, positive anti-Sa effects should be expected especially in those with RA who have high levels of anti-CCP. According to foreign data - showing a large ethnic diversity - the sensitivity of anti-Sa auto antibodies varies between 31-44%, with a specificity of 92-98%. According to some researchers, the presence of anti-Sa / anti-MCV autoantibodies is associated with a more severe course of RA and greater destruction of joints and the level of this marker correlates with disease activity and response to selected drugs. Other researchers do not share the opinion about the diagnostic or prognostic utility of anti-Sa / anti-MCV autoantibodies. Antibodies to citrulline vimentin are usually quantified in serum by immunoassay. Antibodies that capture autoantibodies from the test sample may be citrulline vimentin (so-called antigen Sa) or mutant citrullinated vimentin (MCV). Therefore, this marker - depending on the analytical method used - is referred to as anti-Sa or anti-MCV.
